West Northumberland PCN: Adapt Living Well Coordination Service (Social Prescribing)

Your doctor isn’t the only person who can help you to feel better. The Living Well Coordination Service supports individuals with non-medical issues that may be having a negative impact on their health and wellbeing. This can include issues around housing, finance and benefits, emotional wellbeing, family, relationships and parenting, employment and skills, social networks and healthy lifestyles. As a social prescribing service, we offer support by helping you to identify your needs and linking to a range of services, groups and activities to help meet these needs.

We accept referrals from any member of GP practice staff, partner agencies and self-referrals. 

A referral is for up to eight sessions with a Living Well Coordinator at a pace to suit the individual. It may also be possible to self-refer for additional sessions if needed.

The Living Well Coordination Service is the social prescribing service for the West Northumberland Primary Care Network GP practices. The service is funded by the West Northumberland Primary Care Network and delivered by Adapt (N.E.).

How are services delivered?

The service is open to anyone over the age of 16 who is a registered patient at one of the following GP practices:

- Adderlane Surgery, The Bellingham Practice, Branch End Surgery, Burn Brae Medical Group, Corbridge Medical Group, Haltwhistle Medical Group, Humshaugh and Wark Medical Group, Prudhoe Medical Group, Riversdale Surgery, Scots Gap Medical Group, The Sele Medical Practice and White Medical Group

 

The service can be delivered: By telephone, video call, Face-to-face in GP practicesand in certain community venues.
